fa تأثیر استفاده از پودر ونکومایسین و بتادین برای آماده‌سازی پوست بر پیشگیری از عفونت زخم در شکستگی‌های باز درجه 1 و 2 اندام تحتانی: یک کارآزمایی بالینی دوسوکور The Effect of Using Vancomycin Powder and Betadine for Skin Preparation in the Prevention of Wound Infection in Grades I and II Open Fractures of the Lower Limb: A Double-Blind Clinical Trial جراحی Surgery پژوهشي Original <div style="text-align: justify;">زمینه: آماده‌سازی پوست در اتاق عمل جهت جلوگیری از عفونت پس از جراحی شکستگی باز دارای اهمیت است، لذا مطالعه حاضر با هدف بررسی تأثیر استفاده همزمان بتادین و پودر ونکومایسین برای آماده‌سازی پوست آسیب دیده در پیشگیری از عفونت زخم در شکستگی‌های باز اندام تحتانی تحت جراحی فیکساسیون داخلی انجام شد. مواد و روش‌ها: پژوهش حاضر یک کارآزمایی بالینی دوسوکور بر روی 40 مرد با شکستگی باز اندام تحتانی بود که تحت جراحی جااندازی باز و فیکساسیون داخلی قرار گرفتند. بیماران به طور تصادفی در دو گروه کنترل و مداخله قرار گرفتند. به منظور آماده‌سازی پوست آسیب‌دیده قبل از جراحی، در گروه کنترل از بتادین 10 درصد و در گروه مداخله از بتادین 10 درصد و 2 گرم پودر ونکومایسین استفاده شد. زخم‌های باز ناشی از خروج استخوان از پوست و برش‌های جراحی طی 30 روز (روزهای 1، 2، 14 و 30) پس از جراحی از نظر عفونت با ASEPSIS Index نمره‌گذاری شدند. نتایج با استفاده از نرم‌افزار SPSS ویرایش 22 مورد تجزیه و تحلیل قرار گرفت (0/05p<). یافته‌ها: دو گروه از نظر میانگین سن، مدت زمان بستری اورژانس تا زمان جراحی، قد، وزن و شاخص توده بدنی، وضعیت تأهل، نوع شکستگی، میزان خون‌ریزی، نمره عفونت زخم و زمان جراحی تفاوت معنی‌داری نداشتند (0/05<p). نتیجه‌گیری: استفاده همزمان از بتادین و پودر ونکومایسین بر عفونت زخم پس از جراحی جااندازی باز و فیکساسیون داخلی در بیماران با شکستگی باز اندام تحتانی تأثیری ندارد.</div> <div style="text-align: justify;">Background: Skin preparation in the operating room is important to prevent infection after open fracture surgery. The present study was conducted with the aim of investigating the effect of the simultaneous use of betadine and vancomycin powder for the preparation of damaged skin in preventing wound infection in open fractures of the lower limb undergoing internal fixation surgery. Materials and Methods: The present study was a double-blind clinical trial on 40 men with open fractures of the lower limb who underwent open reduction surgery and internal fixation. Patients were randomly divided into control and intervention groups. In order to prepare the damaged skin before surgery, betadine 10% was used in the control group and betadine 10% plus 2 grams of vancomycin powder in the intervention group. Open wounds caused by the removal of bone from the skin and surgical incisions within 30 days (days 1, 2, 14 and 30) after surgery were scored in terms of infection using the ASEPSIS scoring method. The results were analyzed in SPSS 22 software (p<0.05). Results: There was no significant difference between the two groups in terms of mean age, duration of emergency hospitalization until surgery, height, weight and body mass index, marital status, fracture grade, bleeding rate, wound infection score and surgery duration (p>0.05).
