fa ارتباط مواجهه با فلزات سنگین ارزیابی‌شده از طریق پایش محیطی با اختلال کم‌توجهی- بیش‌فعالی (ADHD): یک مرور نظام‌مند Association between Heavy Metal Exposure Assessed by Environmental Monitoring and ADHD: A Systematic Review بهداشت محیط Environmental Health مرور سیستماتیک و فراتحلیل Review Systematic and Meta Analysis <div style="text-align: justify;">زمینه: نظر به شیوع و مشکلات عدیده افراد مبتلا به اختلال کمبود توجه- بیش فعالی و احتمال نقش عوامل متعدد در بروز این اختلال، اخیراً فلزات سنگین از دسته عوامل خطر محیطی مورد توجه محققان قرار گرفته‌اند. با این‌حال، شواهد موجود در خصوص مواجهه‌های ارزیابی‌شده از طریق پایش محیطی و ارتباط آن با ADHD پراکنده و ناهمگون است. لذا، مطالعه حاضر با هدف بررسی ارتباط مواجهه با فلزات سنگین ارزیابی شده از طریق پایش محیطی با اختلال کم‌توجهی- بیش‌فعالی انجام شد. مواد و روش‌ها: مطالعه حاضر یک مرور نظام‌مند مطابق با دستورالعمل پرایزما بود. جستجو در پایگاه‌های داده Scopus، Web of Science و PubMed انجام شد که به‌دلیل پوشش جامع، نمایه‌سازی گسترده و اعتبار علمی بالا انتخاب شدند. تمامی مقالات انگلیسی زبان منتشر شده تا ۲۰ سپتامبر ۲۰۲۴ بدون محدودیت زمانی جستجو شدند. کلیدواژه‌های مرتبط بر اساس اصطلاحات MeSH استفاده شدند و تمامی مراحل غربالگری عنوان، چکیده و متن کامل مقالات بر اساس معیارهای ورود و خروج توسط دو پژوهشگر مستقل انجام شد. مطالعات مشاهده‌ای که مواجهه با فلزات سنگین را از طریق نمونه‌برداری محیطی (آب، مواد غذایی و سایر منابع محیطی) ارزیابی کرده بودند، وارد مطالعه شدند. در نهایت، استخراج داده‌های مورد نیاز از مقالات نهایی بازیابی شده، انجام شد و کیفیت روش‌شناختی مطالعات وارد شده با استفاده از چک‌لیست اختصاصی طراحی شده بررسی شد. روش تجزیه‌ و تحلیل داده‌ها به‌صورت داده‌های توصیفی انجام شد و به‌دلیل ناهمگنی در یافته‌های مطالعات متاآنالیز امکان‌پذیر نبود. یافته‌ها: از میان ۱۹۴۱۶ نتیجه حاصل از جستجو، پس از حذف ۶۶۹۴ مورد تکراری و انجام غربالگری، در نهایت ۵ مطالعه برای تحلیل نهایی انتخاب شدند. اکثر مطالعات از نوع مشاهده‌ای و در ۵ سال اخیر و عمدتاً در کشورهای اروپایی انجام شده بودند. بیشترین نوع منبع مورد بررسی آلاینده‌ها شامل آب آشامیدنی (۳ مطالعه) و مواد غذایی (۲ مطالعه) بودند. شایع‌ترین فلزات مورد بررسی منگنز و متیل جیوه بودند و پیامدهای مورد ارزیابی شامل تشخیص ADHD، علائم رفتاری مرتبط و زیرگونه‌های این اختلال بود. در مجموع، حدود نیمی از مطالعات (۲ مطالعه) ارتباط مثبت و معناداری بین مواجهه با فلزات سنگین و ADHD گزارش کردند، در حالی‌که دو مطالعه عدم ارتباط معنادار و یک مطالعه ارتباط متفاوت بر اساس زیرنوع ADHD ارائه داد. نتیجه‌گیری: این مرور سیستماتیک نشان داد که شواهد موجود، اگرچه محدود و ناهمگون، از ارتباط بالقوه مواجهه با برخی فلزات سنگین- به‌ویژه منگنز و متیل جیوه- ارزیابی ‌شده از طریق پایش محیطی آب آشامیدنی و دریافت غذایی با بروز یا برخی زیر نوع ADHD حمایت می‌کنند. با این‌حال، تعداد کم مطالعات، تمرکز بر منابع مواجهه محدود و فقدان بررسی مواجهه‌های همزمان، امکان نتیجه‌گیری قطعی را محدود می‌سازد. این یافته‌ها بر اهمیت تقویت نظام پایش محیطی و انجام مطالعات جامع‌تر برای ارزیابی دقیق‌تر ریسک ADHD در کودکان تأکید می‌کند.</div> <div style="text-align: justify;">Background: Given the prevalence and numerous problems of people with attention deficit hyperactivity disorder and the multifactorial nature of this disorder, heavy metals have recently been a focus for researchers as an environmental risk factor. However, the available evidence regarding exposures assessed through environmental monitoring and their association with ADHD is scattered and heterogeneous. Therefore, the present study aimed to investigate the association of exposure to heavy metals assessed through environmental monitoring with attention deficit hyperactivity disorder. Materials and Methods: The present systematic review was performed in accordance with the PRISMA guidelines. A search was conducted in the Scopus, Web of Science, and PubMed databases, selected for their comprehensive coverage, extensive indexing, and high scientific credibility. All English-language articles published up to September 20, 2024 were searched without time limits. Relevant keywords were used based on MeSH terms, and all stages of screening– of the title, abstract, and full text of the articles– were performed based on the inclusion and exclusion criteria by two independent researchers. Observational studies that assessed exposure to heavy metals through environmental sampling (water, food, and other environmental sources) were included in the study. Finally, the required data were extracted from the final retrieved articles and the methodological quality of the included studies was assessed using a specifically designed checklist. The data analysis method was descriptive and meta-analysis was not possible due to heterogeneity in the findings of the studies. Results: Out of 19,416 search results, after removing 6,694 duplicates and performing screening, five articles were finally selected for the final analysis. Most of the studies were observational and conducted in the last five years, mainly in European countries. The most common sources of pollutants studied included drinking water (three studies) and food (two studies). The most common metals studied were manganese and methylmercury, and the outcomes evaluated included ADHD diagnosis, related behavioral symptoms, and subtypes of this disorder. Overall, about half of the studies (2 studies) reported a positive and significant association between heavy metal exposure and ADHD, while two studies reported no significant association and one study reported a different association based on ADHD subtype. Conclusion: This systematic review showed that the available evidence, although limited and heterogeneous, supports a potential association of exposure to some heavy metals– particularly manganese and methylmercury– assessed through environmental monitoring of drinking water and dietary intake with the occurrence or some subtypes of ADHD. However, the small number of studies, the focus on limited exposure sources, and failure to examine concurrent exposures limit the possibility of drawing definitive conclusions.
