fa عملکرد تشخیصی شاخص‌های نیمه کمی تصویربرداری تشدید مغناطیسی داینامیک ۱T برای گریدینگ تومورهای گلیوما قبل از عمل جراحی Diagnostic Performance of Semi-Quantitative Indices of Dynamic T1-Weighted Magnetic Resonance Imaging for Presurgical Glioma Grading رادیولوژی Radiology پژوهشي Original <div style="text-align: justify;">زمینه: بدلیل محدودیت‌های ذاتی روش هیستوپاتولوژی به‌عنوان روش استاندارد برای گریدینگ تومورهای گلیوما، در سال‌های اخیر روش‌های مکمل از جمله روش‌های مبتنی بر داده‌های تصویربرداری برای گریدینگ بهتر تومورهای گلیوما پیشنهاد شده ‌است. این مطالعه با هدف تعیین عملکرد تشخیصی پارامترهای نیمه کمی روش تصویربرداری تشدید مغناطیسی دینامیک با وزن T1 (T1W-MRI) برای گریدینگ تومورهای گلیوما قبل از جراحی انجام شد. مواد و روش‌ها: بیماران مشکوک به تومور مغزی که بین بهمن ماه 1399 تا دی ماه 1401 به بخش تصویربرداری مرکز سرطان مرجع ارجاع داده شده بودند، به صورت گذشته‌نگر بررسی شدند. تعداد ۳۱ بیمار مبتلا به گلیومای تأیید شده توسط نتایج پاتولوژی دارای شرایط ورود به مطالعه بودند. میانگین سنی بیماران 1/14± 39/2 سال بود و ۱۸ نفر از شرکت‌کنندگان مرد (58/6) درصد بودند. پارامترهای نیمه کمی IAUC، Peak و  Initial Slopeبا استفاده از داده‌های T1W-MRI داینامیک کمی‌سازی شدند. از آزمون Mann-Whitney U برای ارزیابی معناداری تفاوت‌ این پارامترها بین گریدهای مختلف تومورهای گلیوما استفاده شد. عملکرد پارامترها برای گریدینگ گلیوما با استفاده از تحلیل منحنی مشخصه عملکرد گیرنده (ROC) ارزیابی شد. یافته‌ها: پارامترهای IAUC، Peak و Initial Slope تفاوت معنی‌داری بین گریدهای مختلف گلیوما داشتند (0/041P≤) و نتایج نشان داد که  IAUCدر مقایسه با سایر پارامترهای مورد مطالعه، بهترین عملکرد گریدینگ را دارد. نتیجه‌گیری: پارامترهای نیمه کمی حاصل از روش T1W-MRI داینامیک اندیس‌های ارزشمندی هستند که می‌توانند به‌طور قابل اعتمادی بین گریدهای گلیوما تمایز قائل شوند. این پارامترهای تصویربرداری فیزیولوژیک غیرتهاجمی می‌توانند به‌عنوان یک مکمل قدرتمند برای سیستم گریدینگ استاندارد هیستوپاتولوژیک عمل کنند.</div> <div style="text-align: justify;">Background: In recent years, complementary methods, including imaging-based approaches, have been proposed for better grading of glioma tumors due to the inherent limitations of histopathology as the standard method. This study aimed to determine the diagnostic performance of semi-quantitative parameters of dynamic T1-weighted magnetic resonance imaging (T1W-MRI) for presurgical glioma grading. Materials and Methods: Patients with brain tumors referred to the imaging department of a referral cancer center between February 2021 and July 2023 were retrospectively reviewed. Thirty-one patients with glioma confirmed by pathology results met the inclusion criteria. The mean age of the patients was 39.2±1.14 years, and 18 participants were male (58.06%). Semi-quantitative parameters (IAUC, Peak, and Initial Slope) were quantified using dynamic T1W-MRI data. The Mann-Whitney U test was used to assess the significance of the difference between these parameters in different grades of glioma. The performance of these parameters for glioma grading was evaluated using receiver operating characteristic (ROC) curve analysis. Results: IAUC, Peak, and Initial slope parameters showed significant differences between different glioma grades (P≤0.041), and the results indicated that IAUC had the best grading performance compared to the other studied parameters. Conclusion: Semi-quantitative parameters quantified using dynamic T1W-MRI are valuable indices that can reliably discriminate between glioma grades.
