fa ویژگی‌های بالینی، الگوهای عود و بقای کودکان مبتلا به تومور ویلمز: مطالعه کوهورت گذشته‌نگر تک‌مرکزی در اهواز، ۱403–۱396 Clinical Characteristics, Recurrence Patterns, and Survival of Children with Wilms Tumor: A Single-Center Retrospective Cohort Study in Ahvaz, 2017–2024 خون و سرطان Blood and cancer پژوهشي Original <div style="text-align: justify;">زمینه: تومور ویلمز شایع‌ترین بدخیمی کلیوی در کودکان است. با وجود پیشرفت‌های درمانی، اطلاعات مربوط به ویژگی‌های بالینی، بقا و الگوی عود آن در ایران، به‌ویژه در جنوب کشور، محدود است. این مطالعه به بررسی ویژگی‌های اپیدمیولوژیک و عوامل مؤثر بر بقا و عود در بیماران مبتلا به تومور ویلمز در بیمارستان بقایی ۲ اهواز پرداخته است. مواد و روش‌ها: این مطالعه توصیفی– تحلیلی با طراحی کوهورت گذشته‌نگر بر روی 50 بیمار زیر 18 سال با تشخیص قطعی پاتولوژیک تومور ویلمز که از سال‌های 1396 تا 1403 در بیمارستان بقایی 2 اهواز بستری و پیگیری شده بودند انجام شد. داده‌های جمعیت‌شناختی، علائم بالینی، روش‌های تشخیصی و درمانی، مرحله بیماری، بقا و عود بررسی شد. تحلیل بقا با روش Kaplan–Meier و مدل رگرسیون Cox انجام گرفت. یافته‌ها: از 50 بیمار، 28 نفر دختر (56 درصد) و 22 نفر پسر (44 درصد) بودند. میانگین سنی بیماران1/2± 4/8 سال بود. شایع‌ترین علامت بالینی، توده شکمی بدون درد بود که در 26 بیمار (52 درصد)؛)95 درصد- 2/8-38/65(CI: مشاهده شد. شایع‌ترین مرحله بیماری، مرحله II بود. همه بیماران تحت نفرکتومی قرار گرفتند؛ شیمی‌درمانی در 46 بیمار (92 درصد) و رادیوتراپی در 26 بیمار (52 درصد) انجام شد. در طول پیگیری، 6 بیمار (12 درصد) دچار عود شدند که بیشتر در مرحله IV بودند. بر اساس تحلیل کاپلان- مایر، میزان بقای ۵ ساله کلی ۹۴ درصد )95 درصد- 100-87/2:CI) برآورد شد. میزان بقای ۵ ساله در مراحل I و II برابر با 96/1 درصد (95 درصد- 100-91/5CI:) و در مرحله IV برابر با 71/4 درصد (95 درصد- 84/2-58/6CI:) بود. در تحلیل رگرسیون کاکس تک‌متغیره، مرحله پیشرفته بیماری با کاهش بقای کلی ارتباط معنی‌دار داشت؛ 3/21= HR 0/004p= 95 درصد- 7/26-1/42CI:) عود در ۶ بیمار (۱۲ درصد) رخ داد که از نظر توصیفی، بیشتر در مراحل پیشرفته مشاهده شد؛ با این حال، به دلیل تعداد محدود رویدادها، تحلیل استنباطی مستقلی برای بررسی عوامل مرتبط با عود انجام نشد. نتیجه‌گیری: مرحله بیماری در زمان تشخیص، مهم‌ترین عامل مرتبط با بقای کلی در این مطالعه بود. اگرچه عود به‌صورت توصیفی در مراحل پیشرفته بیشتر مشاهده شد، به دلیل تعداد محدود موارد عود، درباره ارتباط آماری مستقل مرحله بیماری با خطر عود نمی‌توان نتیجه‌گیری قطعی کرد.</div> <div style="text-align: justify;">Background: Wilms tumor is the most common renal malignancy in children. Despite therapeutic advances, information on its clinical features, survival, and recurrence pattern in Iran, especially in the southern region, remains limited. This study investigated the epidemiological features and factors affecting survival and recurrence in patients with Wilms tumor in Baqaei 2 Hospital in Ahvaz, Iran. Materials and Methods: This retrospective descriptive-analytical cohort study was conducted on medical records of 50 patients under 18 years of age with a definitive pathological diagnosis of Wilms tumor who were hospitalized and followed up in Baqaei 2 Hospital, Ahvaz, from 2017 to 2024. Demographic data, clinical symptoms, diagnostic and therapeutic methods, disease stage, survival, and recurrence were examined. Survival analysis was performed using the Kaplan–Meier method and Cox regression model. Results: Of the 50 patients, 28 were female (56%) and 22 were male (44%), with a mean age of 4.8 ± 1.2 years. The most common clinical presentation was a painless abdominal mass (52%; 95% CI: 38.2–65.8). Based on Kaplan-Meier analysis, the 5-year overall survival rate was estimated to be 94% (95% CI: 87.2–100). The 5-year survival rate in stages I and II was 96.1% (95% CI: 91.5–100), whereas in stage IV, it was 71.4% (95% CI: 58.6–84.2). In univariate Cox regression analysis, advanced disease stage was significantly associated with reduced overall survival; HR = 3.21 (95% CI: 1.42–7.26; P = 0.004). Recurrence occurred in six patients (12%), which was descriptively more common in advanced stages; however, due to the limited number of events, an independent inferential analysis was not performed to examine factors associated with recurrence. Conclusion:. Disease stage at diagnosis was the most important factor associated with overall survival in this cohort.
