زمینه: هلیکوباکتر پیلوری عامل اصلی بیماری‌های مختلف گوارشی می‌باشد. از آنجا که بیماری‌های ناشی از هلیکوباکتر پیلوری با آسیب بافتی شدید همراه است، احتمالاً پروتئازهای باکتری نقش مهمی را در این فرآیند ایفا می‌کنند. در این مطالعه تنوع فعالیت پروتئازی سویه‌های هلیکوباکتر پیلوری و ارتباط آنها با ضایعات مختلف پاتولوژیک معده بررسی شد.

مواد و روش‌ها: این پژوهش بر روی 116 نمونه بیوپسی تهیه شده از بیماران با اختلالات گوارشی مراجعه کننده به بیمارستان آیت الله طالقانی تهران انجام شد. با آزمون کشت و PCR (واکنش زنجیره‌ای پلی‌مراز) حضور هلیکوباکتر پیلوری تشخیص داده شد. سپس فعالیت پروتئولیتیکی این سویه‌ها توسط آزمون اسپکتروفتومتری و چاهک پلیت با استفاده از سوبسترای کازئینی سنجیده شد. از آزمون آنالیز واریانس
(One-way ANOVA) جهت تجریه و تحلیل داده‌ها استفاده شد.

یافته‌ها: شیوع هلیکوباکتر پیلوری با آزمون کشت و PCR، 43/1 درصد شناسایی گردید. فراوانی و نوع ضایعات هیستوپاتولوژی در 50 بیمار، شامل40 درصد (20/50) گاستریت مزمن، 8 درصد (4.50) اینتستینال متاپلاژی و 52 درصد (26.50) گاستریت فعال شدید بود. نتایج مطالعات ما نشان داد که 4/25درصد سویه‌ها فعالیت پروتئازی بالا، 91/4درصد فعالیت متوسط و 4/25درصد باقیمانده فعالیت کم داشتند. آنالیز آماری وجود ارتباط معنی‌دار بین فعالیت پروتئازی پایین سویه‌های هلیکوباکتر پیلوری و گاستریت مزمن را مشخص کرد (0/044=p).

نتیجه‌گیری: نتایج این مطالعه وجود تنوع فعالیت پروتئازی بین سویه‌های مختلف هلیکوباکتر پیلوری و معنی‌دار بودن ارتباط بین فعالیت پروتئازی پایین سویه‌های هلیکوباکتر پیلوری و گاستریت مزمن را نشان داد که مطالعات تکمیلی جهت بررسی این ارتباط لازم می‌باشد.
