fa بررسی نحوه تجویز آنتی‌بیوتیک پروفیلاکسی در اعمال جراحی در بیمارستان آموزشی شهر بوشهر جراحی Surgery پژوهشي Original <div style="text-align: justify;"><strong><u><span style="font-family:B Lotus;"><span style="font-size:11.0pt;">زمینه:</span></span></u></strong><strong><span style="font-family:B Lotus;"><span style="font-size:10.0pt;"> یکی از روش‌های مؤثر در جلوگیری از عفونت محل جراحی استفاده ازآنتی بیوتیک پروفیلاکسی است ولی استفاده نادرست از آنتی‌بیوتیک‌ها باعث ایجاد عفونت‌های مقاوم باکتریایی و عوارض دارویی و زیان‌های اقتصادی می‌شود. در این مطالعه نحوه تجویز منطقی آنتی‌بیوتیک بررسی و نتایج با دستورالعمل‌های بین‌المللی مقایسه شد.</span></span></strong><br> <strong><u><span style="font-family:B Lotus;"><span style="font-size:11.0pt;">مواد و روش‌ها:</span></span></u></strong> <strong><span style="font-family:B Lotus;"><span style="font-size:10.0pt;">در این پژوهش 348 بیمار کاندید جراحی الکتیو در مدت 6 ماه بررسی شدند. اطلاعات دموگرافیک بیماران و نوع عمل جراحی، اندیکاسیون آنتی‌بیوتیک، نوع آنتی‌بیوتیک، دوز، زمان تجویز قبل از عمل و مدت دریافت آنتی‌بیوتیک وارد چک لیست شد و سپس یافته‌ها با دستورالعمل انجمن داروسازان آمریکا مقایسه شد. </span></span></strong><br> <strong><u><span style="font-family:B Lotus;"><span style="font-size:11.0pt;">یافته‌ها:</span></span></u></strong> <strong><span style="font-family:B Lotus;"><span style="font-size:10.0pt;">در بین 348 بیمار در 88/5 درصد موارد اندیکاسیون تجویز پروفیلاکسی به درستی تشخیص داده شده بود. در بین افرادی که اندیکاسیون تجویز پروفیلاکسی داشتند، نوع آنتی‌بیوتیک مورد استفاده قبل از عمل در 73 مورد از87 نفر (8/9&nbsp; درصد) مناسب بود. دوز آنتی‌بیوتیک قبل از جراحی در 72 بیمار از 73 نفر (98/6 درصد) درست بود. زمان تجویز دارو قبل از جراحی در 36 بیمار (49/4 درصد) صحیح بود و از نظر مدت دریافت آنتی‌بیوتیک، در 53 مورد (27/7 درصد) دستورالعمل‌های استاندارد رعایت شده بود.</span></span></strong><br> <strong><u><span style="font-family:B Lotus;"><span style="font-size:11.0pt;">نتیجه‌گیری:</span></span></u></strong><strong><span style="font-family:B Lotus;"><span style="font-size:10.0pt;"> در این مطالعه کمترین همخوانی با دستورالعمل‌های بین‌المللی در طول مدت دریافت پروفیلاکسی مشاهده شد. با توجه به عوارض جانبی مصرف بی‌رویه آنتی‌بیوتیک و بار مالی ناشی از آن، به نظر می‌رسد تدوین یک دستورالعمل واحد، آموزش به تجویزکنندگان برای اجرای دستورالعمل، پیگیری دقیق و دریافت بازخورد از اجرای آن راه حل مطلوب برای حل این معضل باشد.</span></span></strong></div> <div style="text-align: justify;"><strong><u>Background</u></strong><strong>:</strong> One of the most effective modalities for reducing surgical site infection is the use of&nbsp;prophylactic antibiotics, but the inappropriate consumption of antibiotics before operations leads to an&nbsp;increased prevalence of resistant bacterial infections, adverse drug side-effects and financial burden. This study examined the logical administration of prophylactic antibiotics and its patient outcomes and compared the results with international guidelines.<br> <strong><u>Materials and Methods</u></strong><strong>:</strong> In this study, 348 patients who were elective surgery candidates were assessed over six months. The patients&rsquo; demographic information, type of operation, and antibiotic indication, type, dosage, and time and duration of administration before the operation were entered into a checklist and the results were compared against the American Society of Health system Pharmacists (ASHP) guidelines.<br> <strong><u>Results:</u></strong> The indication for prophylactic antibiotics was correct in 88.5% of the 348 patients examined. In those with prophylactic antibiotic indication, the antibiotic type used before surgery was appropriate in 73 (83.9%) out of 87 cases. The antibiotic dosage before the surgery was correct in 72 (98.6%) patients. The timing of antibiotic administration was correct in 36 (49.4%) cases. The duration of antibiotic administration was in accordance with the standard guidelines in 53 patients (27.7%).<br> <strong><u>Conclusion</u></strong><strong>:</strong> In this study, the lowest accordance with standard guidelines was observed in the duration of&nbsp;prophylactic antibiotic intake.
