حساسیت آنتیبیوتیکی میکروارگانیسمهای عفونتزای دستگاه ادراری در بیماران بزرگسال سرپایی در شهر بوشهر

<p align="justify">زمینه: عفونت دستگاه ادراری یکی از شایع ترین بیماری های عفونی شناخته شده در بیماران سرپایی و بستری در بیمارستان است. از این رو شناخت عوامل مولد این عفونت و درمان صحیح آن، از به وجود آمدن عوارض مربوط به این بیماری جلوگیری می کند. هدف از مطالعه حاضر, شناخت حساسیت آنتی بیوتیکی میکروارگانیسم های شایع به وجود آورنده عفونت ادراری در شهر بوشهر در سال 1385 است. مواد و روشها: در این مطالعه گذشته نگر، نمونه ادراری 2000 بیمار مراجعه کننده به صورت سرپایی به آزمایشگاه های معتبر سطح شهر بوشهر در سال 1385 جمع آوری گردید و نتایج حاصل از کشت ادراری پس از مطابقت با نتایج آنالیز ادراری و حذف موارد با آنالیز ادراری نرمال، مورد استفاده قرار گرفت و نتایج حاصل از سنجش حساسیت ضد میکروبی عوامل مولد عفونت ادراری که به روش disc diffusion انجام شده بود, ثبت گردید. یافتهها: شیوع عفونت ادراری در زنان (8/85 درصد) بالاتر بود. شایعترین عوامل مولد عفونت ادراری، اشرشیاکلی (2/40 درصد) و سپس استافیلوکوکها (7/23 درصد) بودند؛ سایر ارگانیسمها شامل کلبسیلا (8/20 درصد) و انتروباکتر (0/12 درصد) به ترتیب ردههای بعدی را به خود اختصاص دادند. در 4/3 درصد از بیماران، گروهی از میکروارگانیسمها شامل سراشیا، سودوموناس، سیتروباکتر، اسینتوباکتر و پروتئوس جدا شدند که تحت عنوان گروه متفرقه نام گذاری شدند. اشرشیاکلی که شایع ترین عامل مولد عفونت ادراری بود، بیشترین حساسیت ضد میکروبی را نسبت به نسل 3 سفالوسپورینها (1/88 درصد) و بیشترین مقاومت را نسبت به وانکومایسین (1/97درصد) داشته است. گروه استافیلوکوکها حساسیت قابل توجهای نسبت به آمینوگلیکوزیدها (3/94 درصد) و بیشترین میزان مقاومت را نسبت به نسل 3 سفالوسپورینها (4/67 درصد) از خود نشان داد. نتیجه گیری: شایعترین عامل مولد عفونت ادراری در شهر بوشهر اشرشیاکلی است که حساسیت قابل توجهی نسبت به نسل 3 سفالوسپورینها دارد و دومین ارگانیسم شایع که استافیلوکوکها هستند نیز بالاترین حساسیت ضد میکروبی را نسبت به آمینوگلیکوزیدها دارند. </p>
Background: Urinary tract infection (UTI) is one of the most common infectious diseases in inpatients and outpatients, thus recognition of the pathogen and a proper treatment can prevent related complications. The aim of this study was to identify various microorganisms causing community acquired UTI and their relative prevalence and antibiotic susceptibilities in outpatients of Bushehr port in 2006. Methods: In this retrospective study, urine samples of 2000 outpatients with community acquired UTI who referred to standard clinical laboratories of Bushehr in the year of 2006 were collected. The results of urine cultures were adjusted to the results of Urine analysis. The microbial susceptibilities of urinary tract infection pathogens were evaluated using disc diffusion method. Result: UTI in women was more prevalent (85.8%). The most common UTI pathogen was Escherichia coli (40.2%) and staphylococci (23.7%), respectively. Other organisms were Klebsialla and Entrobacter (20.8% and 12%, respectively). In 3.4% of patients, Serratia, Pseudomonas, citrobacter, Proteus mirabilis and Acinetobacter were isolated, as miscellaneous group. Escherichia coli (88.1%), which is the most common cause of UTI had the highest antibiotic sensivity to the third generation of cephalosporins. and the highest antibiotic resistance to vancomycin (97.1%). Staphylococci showed a high susceptibility to Aminoglycosides (94.3%) and the most resistance to the third generation of cephalosporins (67.4%). Conclusion: The most common cause of UTI in Bushehr port was Escherichia coli with a high susceptibility to the third generation of cephalosporins. The second common organisms were staphylococci with a high antimicrobial susceptibility to aminoglycosides.
