fa ارتباط ماندگاری بیماران مبتلا به HIV در برنامه‌های مراقبتی با میزان بروز بیماری‌های همراه و کاهش بار ویروسی Retention in HIV Care Programs and Its Association with Comorbidity Incidence and Viral Load Reduction بیمارستان‌ها و دیگر مراکز بهداشتی Hospitals and Other Health Facilities پژوهشي Original <div style="text-align: justify;">زمینه: ویروس نقص ایمنی انسانی (HIV) یکی از چالش‌های مهم بهداشت عمومی در سطح جهانی است که با افزایش بار بیماری‌های همراه و کاهش کیفیت زندگی مبتلایان مرتبط است. این مطالعه با هدف بررسی ارتباط بین ماندگاری بیماران HIV مثبت در برنامه‌های مراقبتی و بروز بیماری‌های همراه و همچنین تأثیر آن بر کاهش بار ویروسی انجام شد. مواد و روش‌ها: این مطالعه توصیفی- تحلیلی بر روی ۱۹۰ بیمار HIV مثبت (از مجموع ۲۰۲ پرونده فعال) بالای ۱۸ سال انجام شد که بین سال‌های ۱۳۹۶ تا ۱۴۰۱ در مرکز مشاوره بیماری‌های رفتاری خرمشهر تحت پیگیری قرار داشتند. اطلاعات دموگرافیک، ویژگی‌های درمانی و داده‌های بالینی از طریق بررسی پرونده‌های پزشکی جمع‌آوری شد. تحلیل داده‌ها با نرم‌افزار SPSS ویرایش 28 و استفاده از آزمون‌های آماری مانندANOVA  و ضریب همبستگی پیرسون انجام گرفت. یافته‌ها: 68/4 درصد بیماران دارای ماندگاری بالا (بیش از 70 درصد ویزیت‌های برنامه‌ریزی شده)، 26/3 درصد نیمه‌ماندگار (40 و 70 درصد ) و 5/3 درصد دارای ماندگاری پایینی (کمتر از 40 درصد) بودند. بیماران با ماندگاری بالاتر، سطح CD4 بیشتر و بار ویروسی کمتری داشتند. وضعیت اقتصادی و تحصیلات بالاتر با سطح CD4 بالاتر و بار ویروسی کمتر مرتبط بود. بیش از نیمی از بیماران (55/3 درصد) حداقل به یک بیماری همراه مبتلا بودند که شایع‌ترین آن‌ها هپاتیت B، هپاتیت C و سل بودند. نتیجه‌گیری: ماندگاری در برنامه‌های مراقبتی HIV با بهبود وضعیت ایمنی (افزایش سطح CD4) و کاهش بار ویروسی همراه است. این مطالعه بر اهمیت طراحی و اجرای برنامه‌های حمایتی جامع برای بیماران HIV مثبت به‌منظور افزایش ماندگاری در درمان، مدیریت بیماری‌های همراه و بهبود کیفیت زندگی آنان تأکید دارد.</div> <div style="text-align: justify;">Background: The human immunodeficiency virus (HIV) remains a critical global public health challenge, contributing to a growing burden of comorbidities and diminished quality of life among affected individuals. This study aimed to examine the association between retention in HIV care programs and the occurrence of comorbidities, as well as its impact on viral load suppression. Materials and Methods: This descriptive-analytical study included 190 HIV-positive patients (out of 202 active records) aged over 18 years who were monitored at the Khorramshahr Behavioral Diseases Counseling Center between 2017 and 2022. Demographic data, treatment characteristics, and clinical information were extracted from medical records. Data were analyzed using SPSS software, with statistical tests including ANOVA and Pearson’s correlation coefficient. Results: The study found that 68.4% of the patients had high retention (attending>70% of scheduled visits), 26.3% had moderate retention (40–70%), and 5.3% had low retention (<40%). Higher retention was associated with increased CD4 counts and lower viral loads. Additionally, better socioeconomic status and higher education levels correlated with improved CD4 levels and reduced viral loads. Over half of the patients (55.3%) had at least one comorbidity, with hepatitis B, hepatitis C, and tuberculosis being the most prevalent. Conclusion: Retention in HIV care programs is linked to enhanced immune function (higher CD4 counts) and improved viral suppression.
