fa بررسی مولکولی ژن‌های پمپ افلاکس AdeIJK در اسینتوباکتر بومانی با مقاومت دارویی گسترده و ارتباط آن با الگوی مقاومت ضدمیکروبی Molecular Investigation of AdeIJK Efflux Pump Genes in Extensively Drug Resistant Acinetobacter Baumannii and their Associa-tion with Antimicrobial Resistance Patterns باکتریولوژی Bacteriology پژوهشي Original <div style="text-align: justify;">زمینه: اسینتوباکتر بومانی به علت دارا بودن انواعی از عوامل مؤثر در مقاومت آنتی‌بیوتیکی مانند پمپ‌های افلاکس، آنزیم‌های بتالاکتامازی و توانایی تشکیل بیوفیلم قوی، مشکلات درمانی متعددی را در بیماران ایجاد می‌کند. این مطالعه با هدف بررسی میزان حضور ژن‌های پمپ‌ افلاکس AdeIJK در جدایه‌های بالینی با مقاومت دارویی گسترده اسینتوباکتر بومانی از شهر قم و ارزیابی ارتباط آن با مقاومت‌های ضدمیکروبی انجام شد. مواد و روش‌ها: در این مطالعه توصیفی– مقطعی، در 81 جدایه اسینتوباکتر بومانی با مقاومت دارویی گسترده، حضور ژن‌های adeI، adeJ، adeK به روش PCR بررسی شد. سپس ارتباط حضور هر یک از ژن‌های مذکور با مقاومت ضدمیکروبی جدایه‌ها مورد ارزیابی قرار گرفت. نتایج بدست آمده با نرم‌افزار SPSS ویرایش 16 و آزمون پیرسون مورد تجزیه و تحلیل قرار گرفت. یافته‌ها: در تحقیق حاضر میزان مقاومت جدایه‌ها در برابر پلی‌میکسین، کولیستین و توبرامایسین به‌ترتیب 15/6 درصد، 42/74 درصد و 48/9 درصد مشاهده شد. در بین 81 جدایه اسینتوباکتر بومانی با مقاومت دارویی گسترده، 69 جدایه (85/18 درصد) دارای ژن adeI، 76 جدایه (93/82 درصد) دارای ژن adeJ و 71 جدایه (87/65 درصد) دارای ژن adeK بودند. همچنین 95 درصد جدایه‌ها حداقل واجد دو ژن از پمپ AdeIJK  بودند که با فنوتیپ مقاومت دارویی گسترده جدایه‌ها هماهنگ می‌باشد. نتیجه‌گیری: نتایج نشان داد که بیش از 97 درصد جدایه‌های اسینتوباکتر بومانی با مقاومت دارویی گسترده به اغلب آنتی‌بیوتیک‌ها مقاوم بودند و بیش از 80 درصد آن‌ها واجد ژن‌های پمپ  AdeIJKنیز بودند. بنابراین اتخاذ استراتژی‌های مناسب برای کنترل و درمان عفونت‌های بیمارستانی جهت جلوگیری از شیوع این ژن‌ها ضروری است.</div> <div style="text-align: justify;">Background: Acinetobacter baumannii causes numerous therapeutic problems in patients due to the presence of various effective factors that facilitate antibiotic resistance such as efflux pumps, beta-lactamase and the ability to form a strong biofilm. The aim of this study was to investigate the presence of AdeIJK efflux pump genes in clinical isolates of A. baumannii with extensive drug resistance and evaluation of its relationship with antimicrobial resistance. Materials and Methods: This descriptive cross-sectional study examined 81 clinical isolates of A. baumannii with extensive drug resistance for the presence of adeI, adeJ, and adeK genes using PCR. Then, the relationship between the presence of each of these genes and the antimicrobial resistance of the isolates was evaluated. The results were analyzed using Pearson's chi-squared test in SPSS software (Version 16). Results: In the present study, the rates of resistance to polymyxin, colistin and tobramycin were observed to be 15.6%, 42.74%, and 48.9%, respectively. Among 81 extensively drug-resistant A. baumannii isolates, 69 isolates (85.18%) carried adeI gene, 76 isolates (93.82%) carried adeJ gene, 71 isolates (87.65%) carried adeK gene. Moreover, 95% of the isolates had at least two genes from the AdeIJK efflux pump, which is consistent with the extensively drug-resistant phenotype of the isolates. Conclusion: The results showed that more than 97% of Acinetobacter baumannii isolates with extensive drug resistance were resistant to most antibiotics and more than 80% of them also had AdeIJK pump genes.
