fa مختل‌کننده‌های اندوکرینی و مشکلات کمبود توجه- بیش‌فعالی: یک مرور نظام‌مند بر مطالعات پایش محیطی مواجهه Endocrine Disruptors and Attention Deficit Hyperactivity Disorder: A Systematic Review of Environmental Monitoring Exposure Studies دستگاه اعصاب Nervous System مرور سیستماتیک و فراتحلیل Review Systematic and Meta Analysis <div style="text-align: justify;">زمینه: اختلال کمبود توجه- بیش‌فعالی (ADHD) یکی از شایع‌ترین اختلالات عصبی- رشدی در کودکان است که علت‌شناسی آن چندعاملی و پیچیده است. شواهد اخیر حاکی از نقش ترکیبی عوامل، به‌ویژه ترکیبات شیمیایی مختل‌کننده غدد درون‌ریز (EDCs)، به‌عنوان عوامل محیطی مؤثر در بروز این اختلال از طریق برهم‌زدن مسیرهای تنظیم هورمونی و رشد عصبی است. با وجود رشد مطالعات در این حوزه، هنوز شواهد کافی درباره رابطه بین مواجهه محیطی با EDCها (بر اساس پایش محیطی) و بروز ADHD وجود ندارد. از این‌رو، این مرور نظام‌مند با هدف تعیین شواهد موجود درباره ارتباط بین مواجهه از طریق پایش محیطی با EDCها و بروز علائم ADHD انجام شد. مواد و روش‌ها: مطالعه حاضر یک مرور نظام‌مند انجام شده مطابق با چک‌لیست پرایزما می‌باشد. تمامی مقالات پژوهشی به زبان انگلیسی بدون محدودیت زمانی چاپ تا ۲۲ سپتامبر ۲۰۲۴ میلادی در سه پایگاه داده Scopus، Web of Science و PubMed با استفاده از کلیدواژه‌های مرتبط با موضوع و بر اساس اصطلاحاتMeSH  جستجو شدند. تمامی مراحل غربالگری مقالات بر اساس معیارهای ورود و خروج انجام شد. در نهایت، داده‌های مورد نیاز از مقالات نهایی بازیابی شده، استخراج شد. یافته‌ها: از میان ۹۰۴۵ نتیجه جستجو، با حذف ۳۴۹۵ مورد تکراری و پس از بررسی کامل متون در نهایت ۸ مقاله برای تحلیل نهایی انتخاب شدند. بیشترین دامنه انتشار مقالات منتخب در ۱۰ سال اخیر بودند و از نظر منابع مواجهه به‌ترتیب هوا، غذا و آب شرب شایع‌ترین منبع بررسی شده بودند، در حالی‌که برای خاک و رسوبات هیچ مطالعه‌ای یافت نشد. بیشترین آلاینده‌های مورد ارزیابی شامل PAHها، بنزن و BTEX بودند. در حدود نیمی از مطالعات، ارتباط مثبت و معنی‌دار بین مواجهه با برخی از EDCها و بروز علائم ADHD گزارش شد، در حالی‌که سایر مطالعات چنین ارتباطی را نیافتند. نتیجه‌گیری: یافته‌های این مرور نشان می‌دهد که اگرچه شواهد رو به رشدی از ارتباط بالقوه بین مواجهه محیطی با EDCها و افزایش خطر ADHD وجود دارد، اما تعداد مطالعات محدود و وجود برخی ناهمگنی‌ها مانع از نتیجه‌گیری قطعی می‌شود. این مطالعه ضرورت طراحی پژوهش‌های آینده با رویکرد طولی و ارزیابی مواجهه چندمنبعی با کنترل متغیرهای مخدوش‌کننده را برجسته می‌سازد. همچنین، توجه به منابع کمتر بررسی‌شده مانند خاک و رسوبات می‌تواند به درک جامع‌تر از مسیرهای واقعی مواجهه و پیامدهای رشد عصبی کودکان کمک کند.</div> <div style="text-align: justify;">Background: Attention deficit hyperactivity disorder (ADHD) is one of the most common neurodevelopmental disorders in children, with a multifactorial and complex etiology. Recent evidence suggests a combined role of factors, especially endocrine-disrupting chemicals (EDCs), as effective environmental factors in the development of this disorder by disrupting hormonal regulatory pathways and neurodevelopment. Despite the growth of studies in this field, there is still insufficient evidence about the relationship between environmental exposure to EDCs (based on environmental monitoring) and the development of ADHD. Therefore, this systematic review was conducted to determine the existing evidence about the relationship between exposure through environmental monitoring to EDCs and the development of ADHD symptoms. Materials and Methods: The present study is a systematic review conducted in accordance with the PRISMA checklist. All research articles in English without publication time limit until September 22, 2024 were searched in three databases: Scopus, Web of Science, and PubMed using keywords related to the topic and based on MeSH terms. All stages of screening of articles were performed based on inclusion and exclusion criteria. Finally, the required data were extracted from the final retrieved articles. Results: Out of 9045 search results, 3495 duplicates were removed and after a complete review of the literature, 8 articles were finally selected for the final analysis. The largest publication range of the selected articles was in the last 10 years and in terms of exposure sources, air, food and drinking water were the most common sources examined, respectively, while no studies were found for soil and sediment. The most frequently evaluated pollutants included PAHs, benzene, and BTEX. About half of the studies reported a positive and significant association between exposure to some EDCs and the occurrence of ADHD symptoms, while other studies did not find such an association. Conclusion: The findings of this review indicate that although there is growing evidence of a potential association between environmental exposure to EDCs and an increased risk of ADHD, the limited number of studies and the presence of some heterogeneity prevent definitive conclusions. This study highlights the need for future longitudinal research designs and multi-source exposure assessments that control for confounding variables.
