نشانگان بالینی و درمان آسیب با ونوم عروس دریایی؛ یک مطالعه مروری ساختار یافته

<p>زمینه: شاخه سنیداریا (Cnidaria) شامل بیش از نه هزار گونه است که تقریباً 100 گونه آن برای انسان خطرساز تلقی میشوند و سالیانه در سراسر جهان مرگ و میر قابل توجهی به دنبال گزش با آنها گزارش گردیده است. علایم بسته به گونه و نوع زهر متفاوتند که شامل تظاهرات موضعی و سیستمیک میباشند. روشها و ترکیبات گوناگونی جهت درمان گزش با عروس های دریایی مورد بررسی قرار گرفتهاند. با توجه به نبود مطالعه ای مروری و نظاممند جامع جهت بررسی تظاهرات بالینی ناشی از گزش و همچنین روشهای درمانی آنها، این مطالعه انجام گردید. مواد و روشها: با جستجوی واژه عروس دریایی Jellyfish در بانک اطلاعات پزشکی PubMed، 1677 مقاله یافت شد و 400 مقاله در زمینه توکسینولوژی در گستره پزشکی انتخاب و به سه گروه اصلی بالینی، زیست پزشکی و بیوتکنولوژیک تقسیمبندی گردیدند و از این میان، مقالات گروه پزشکی با مجموع 222 مقاله، مشتمل بر زیر گروههای تظاهرات سیستمیک (78 مقاله)، تظاهرات پوستی (59 مقاله) و درمان (85 مقاله)، مورد مطالعه سیستماتیک قرار گرفتند. یافتهها: علائم و نشانگان بالینی پس از گزش با عروسهای دریایی عمدتاً به گونه و نوع زهر آنها بستگی دارد، لکن یافتهها عموماً بهصورت تظاهرات پوستی گستردهای بهصورت ضایعات کهیری، اریتم، تاول، تورم و تظاهرات درمونکروتیک شدید، پاپول و ماکولهای سوراخدار، راشهای خطی هایپر پیگمانته و یا پاپولهای کهیری خارشدار بروز مینمایند. علایم سیستمیک پس از آسیب با رزمناو پرتقالی Physalia physalis و عروس دریایی جعبهای (مانند Chironex fleckeri) و در سندرم ایروکنجی بهصورت آشکار مشاهده میشوند. اساس درمان در آسیب با عروسهای دریایی بر پایهی سه اقدام کاستن اثرات موضعی زهر (درد و تخریب بافتی)، پیشگیری از آزاد سازی بیشتر نماتوسیتهای زهری و نیز کنترل واکنشهای سیستمیک میباشند. کاربرد آب گرم (42 درجه برای 20 دقیقه)، پکهای یخی، ضد دردهای موضعی و تزریقی، کورتیکواستروئیدها و آنتی هیستامین ها، اساس درمان و همچنین استفاده از سرکه تجارتی یا اسیداستیک 4 تا 6 درصد و شستن ناحیه گزش با آب دریا برای غیر فعال کردن نماتوسیتها از شایعترین روشهای درمانی محسوب میشوند. آنتی ونوم علیه عروسهای دریایی میتواند اثر ونوم را خنثی کند. نتیجهگیری: نشانگان پوستی از شایعترین علائم بالینی پس از گزش با عروسهای دریایی هستند. هر چند هنوز کارآزماییهای بالینی دو سویه کور کنترل شدهای، در مورد شیوههای درمانی گزش با عروس دریایی انجام نگرفته است، ولی استفاده از سرکه تجاری یا اسیداستیک 6-4 درصد،آب گرم، پک یخی و شستشو با آب دریا از پایههای درمانی گزش با عروسهای دریایی محسوب میشوند.</p>
<p>Background: The phylum Cnidarians have over nine thousand species that approximately, one hundred species are dangerous for humans. Annually, a large number of deaths were reported due to jellyfish stings. The manifestations depend on their species and kind of venoms, and include the local and systemic manifestations. A number of methods and compounds were used and under investigation for management of injuries with jellyfishes. Due to the lack of an integrated systematic review, the current study was done. Materials and Methods: The PubMed data bank was searched for the term “Jellyfish”. A total of 1677 papers were found. These papers were divided into three categories: medical, biomedical and biotechnological fields. The medical category was further divided into three subcategories comprising systemic manifestations, cutaneous manifestations and treatments for the stings of jellyfishes. The biomedical category was further subdivided into genomics, proteomics, and biology of venoms, mechanisms of actions and products of biomedical significance. In this part of systematic review, the medical aspects of injuries with jellyfishes were evaluated. Results: The clinical manifestations in jellyfish envenomation depend on their species and the nature of venoms. The most common clinical manifestations of jellyfish stings are cutaneous presentations like urticasia, erythema, swelling, vesicles and severe dermonectoric manifestations. Systemic manifestations were seen in the stings of box jellyfishes, Portuguese man-of-war and in Irukandji syndrome. The most common recommendations for jellyfish envenomation managements include decreasing the local effects of venom, prevention of the venomous nematocysts release, and Controlling of systemic reactions. Application of commercial vinegar (4 - 6% acetic acid), hot water immersion (HWI) (42 ° C for 20 minutes), ice packs, sea water rinsing for inactivating nematocysts, administration of topical and parenteral analgesics, corticosteroids , and antihistamines are the basics for treatments of jellyfish envenomations. The Anti-venom for jellyfishes can counteract the effects of venom. Conclusion: The cutaneous manifestations in jellyfish envenomation are the most common clinical presentation. There was no any conducted double-blind and controlled clinical trials for the management of jellyfish envenomation, the application of commercial vinegar (4 - 6% acetic acid), hot water, ice packs and sea water rinsing are the fundamentals of treatments in the stings of the jellyfishes.</p>
