خصوصیات دموگرافیک و بالینی بیماران مبتلا به صرع میوکلونیک جوانان در اصفهان

زمینه: صرع میوکلونیک جوانان از شایعترین انواع صرعهای ژنرالیزه ایدیوپاتیک است و حدود 11- 5 درصد کلّ صرعها را تشکیل میدهد. با توجه به خصوصیات بالینی صرع میوکلونیک جوانان، این بیماری درکلینیکها به خوبی تشخیص داده نمیشود و درمان صحیح انجام نمیگیرد و این در صورتی است که پاسخ به درمان در این بیماران عالی و بیماران نیاز به درمان را در تمام طول زندگی دارند.
مواد و روشها: در یک مطالعه توصیفی آینده نگر، از میان بیماران مبتلا به صرع که به درمانگاههای صرع دانشگاه علوم پزشکی اصفهان مراجعه میکردند، بر اساس تابلو بالینی و یافتههای نوار مغز، بیماران مبتلا به صرع میوکلونیک جوانان انتخاب شدند. خصوصیات بالینی، دموگرافیک و درمانی آنها در پرسشنامه مخصوص ثبت و هر 3 ماه به طور منظم و به مدت یکسال مورد پیگیری قرار گرفتند.
یافتهها: از بین 985 بیمار مبتلا به صرع، 50 بیمار (0/5 درصد) صرع میوکلونیک جوانان داشتند. تعداد زنان 5/3 برابر مردان بود و متوسط سن شروع حملات، 13 سالگی بود. سابقه فامیلی صرع، 38 درصد و فاکتورهای تسهیل کننده تشنج در 66 درصد موارد مثبت بود که 79 درصد این فاکتورها شامل خستگی ناشی از فعالیت، استرس، بیخوابی و نورهای فلاشدار بود. تمام بیماران تحت درمان داروئی قرار گرفتند که حملات میوکلونیک و تشنج در 84 درصد به طور کامل و در 14 درصد به طور نسبی کنترل شد و تنها 2 درصد موارد مقاوم به درمان وجود داشت.
نتیجهگیری: با وجود شیوع بالای بیماری در جنس مؤنث و با توجه به مطالعات قبلی و ماهیت ژنتیکی بیماری، لازم است بررسیهای وسیعتر و مطالعات ژنتیکی در این مورد در کشور ما بعمل آید. بیماری از نظر سایر خصوصیات، مشابه مطالعات قبلی بود.

Background: Juvenile myoclonic epilepsy (JME) is one of the most common idiopathic generelized seizures that include about 5-11% of epilepsies. According to clinical features of JME, this disease could not be diagnosed correctly in clinics which results in inappropriate treatment. However, the clinical response of JME to treatment is excellent and the patients need longlife treatment. Methods: In a prospective observational study, JME subjects were selected among epileptic patients referred to neurological clinic of Isfahan University of Medical Sciences, using clinical and electroencephalographical criteria. The clinical, demographic and therapeutic treatment characteristics were written in special questionnaires. Patients were followed every 3 months for one year. Results: Among 985 epileptic patients, 50 (5%) subjects with JME were found. Female to male ratio was 3.5. The mean onset age of the seizure was 13 years. Familial history was positive in 38% of them and the precipitating factors were found in 66%. Stress, flashing lights, somnolence, and fatiguability were the precipitating factors in 79% of occasions. All of patients were treated. Finally, 84% of patients responded to treatment completely and 14% of subjects responded partially and only 2% of them were refractory to treatment. Conclusion: JME is more prevalent in female subjects in Iran however the other clinical features of this form of epilepsy are compatible with the previous international studies. More evaluations including genetic investigation is recommended for JME in Iran.
