زمینه: پیروی از رژیم‌های درمانی مراقبتی، جهت دستیابی به ارتقاء سلامت، بهبود کیفیت زندگی و مراقبت‌های بهداشتی مقرون به صرفه بسیار حیاتی است. با این حال بار بیماری‌های مزمن به‌علت عدم پیروی در سطح جهان همچنان رو به رشد است. شناخت چالش‌های درک ‌شده، می‌تواند موجب تسهیل برنامه‌ریزی برای افزایش پیروی گردد. این مطالعه با هدف تبیین چالش‌های درک‌شده بیماران مزمن در عدم پیروی از رژیم‌های تجویزی انجام شد.

مواد و روش‌ها: مطالعه حاضر، با رویکرد کیفی و روش تحلیل محتوا گرانهیم و لاندمن (Graneheim & Lundman) در سال 1394 انجام شد. 34 مشارکت‌کننده مبتلا به بیماری‌های مزمن شایع، به‌صورت هدفمند انتخاب و وارد مطالعه شدند. داده‌ها از طریق مصاحبه‌های نیمه ساختار عمیق جمع‌آوری و همزمان با جمع‌آوری داده‌ها، تحلیل آنها به‌صورت مقایسه‌ای با استفاده از روش تحلیل محتوا کیفی قراردادی انجام گرفت. 

یافته‌ها: داده‌ها منجر به استخراج چهار درون‌مایه‌ اصلی، شامل نقص در امکان‌پذیری رعایت توصیه‌ها به دنبال از خودگذشتگی برای خانواده، ناکامی در مواجهه با تعارض کار و توصیه‌ها، شکست در تداوم و ناتوانی در انطباق خود با توصیه‌ها، بار منفی و سنگین هزینه‌های رژیم‌های تجویزی و حصار القایی و محدودکننده باورها گردید. این درونمایه‌ها بیانگر ماهیت چالش‌ها در ابعاد فردی، خانوادگی و اجتماعی (فرهنگی و اقتصادی) بیماران مزمن در پیروی از رژیم‌های مراقبتی درمانی بود.

نتیجه‌گیری: یافته‌ها نشان داد که چالش‌های درک‌ شده از تنوع و پیچیدگی خاصی برخوردار است، اگرچه بعضاً درونی و فردی  هستند، اما برخی از آنها بیرونی و خارج از اراده و کنترل شخص است. لذا لازم است سیاستگذاران و برنامه‌ریزان نظام سلامت با توجه به ماهیت، تنوع و پیچیدگی این موانع درک و شناخته ‌شده برای تسهیل در روند پیروی بیماران از رژیم‌های مراقبتی درمانی برنامه‌ریزی و مداخله نمایند.
