عوامل خطر حملات تب و تشنج در کودکان بیمارستان دانشگاهی فاطمه زهرا (س) بوشهر

زمینه: گرچه عمده موارد تب و تشنج، خوش خیم هستند، درک عامل زمینهای آن از نظر بالینی اهمیت دارد. ما در این مطالعه، مشخصات تب و تشنج و نیز عوامل مرتبط با عود و تکرار آن طی بستری بیمارستانی را در کودکان مبتلا به تب و تشنج بررسی نمودیم.
مواد و روشها: تعداد 102 کودک دچار حمله تب و تشنج که به صورت سریال در بیمارستان دانشگاهی فاطمه زهرا (س) بوشهر بستری شده بودند مورد مطالعه قرار گرفتند. اطلاعات دموگرافیک و دادههای مربوط به شرح حال و معاینه بیماران در بدو ورود ثبت گردیدند. کودکان کمتر از 5 سال دارای تب بیش از 8/37 درجه سانتیگراد یا شرح حال تب در همراهی با تشنج و رد تشخیص مننژیت یا انسفالیت بر اساس پونکسیون کمری یا تشخیص بالینی وارد مطالعه شدند.
یافتهها: میانگین سنی 1/13± 24 ماه بود، بیشترین شکل تشنـج، تونیک کلونیک (5/60 درصد) و کمترین آن آتونیک و میوکلونیک (هر یک 9/7 درصد) بود. تشنج فوکال در 0/13 درصد و تشنج کمپلکس در 5/32 درصد بیماران دیده شد. سابقه خانوادگی تشنج ناشی از تب 0/59 درصد بود. جنسیت دختر (خطر نسبی 68/2، 95 درصد فاصله اطمینان از 20/1 تا 99/5) سابقه قبلی تب و تشنج (خطر نسبی 56/2، 95 درصد فاصله اطمینان 11/1 - 95/5)، سن بروز اولین حمله تب و تشنج پایین (خطر نسبی 7/2، 95 درصد فاصله اطمینان از 16/1 تا 29/6) و انواع کمپلکس تشنج (خطر نسبی 86/3، 95 درصد فاصله اطمینان از 1 تا 97/14) خطر حملات مجدد تب و تشنج را طی سیر بیمارستانی افزایش میدادند (05/0>P).
نتیجهگیری: جنسیت، سابقه قبلی تب و تشنج، سن بروز تشنج اولیه پایین و انواع کمپلکس تشنج، خطر حمله مجدد را طی بستری بیمارستانی افزایش میدهند. مطالعات آینده جهت بررسی اثر دراز مدت این عوامل بر عود میتواند به شناسایی کودکانی که نیازمند توجه پزشکی بیشتری در طولانی مدت هستند کمک نماید.

Background: Although the majority of febrile seizures are benign, it is clinically important to understand the underlying mechanism. In this study, we investigated characteristics of febrile convulsion and factors related to its recurrence and its relapse during a hospital course. Methods: A total of 102 patients with febrile seizure who were admitted serially at Fatemeh Zahra (S) University Hospital in Bushehr port were investigated. Demographic information and data of the history and physical examination were recorded. Children under the age of five with temperature of more than 37.8C or with history of fever accompanied by seizure entered the study those with meningitis or encephalitis according to the results of lumbar puncture or clinical diagnosis were ruled out. Results: Mean age of patients was 24±13.1 months, the most frequent type of convulsion was tonic-clonic (60.5%), and the least were atonic and myoclonic (each 7.9%). Focal convulsion was determined in 13.0% and complex type was observed in 32.5% of patients. Family history of febrile seizure was found in 59.0%. Female sex (RR=2.68, 95% CI: 1.20-5.99), history of previous febrile seizure (RR=2.56, 95% CI: 1.11-5.95), age at initial febrile seizure (RR=2.7, 95% CI: 1.16-6.29), and complex type of seizure (RR=3.86, 95% CI: 1-14.97) increased the risk of seizure relapses in a hospital course (P<0.05). Conclusion: Gender, history of previous febrile seizure, age at initial febrile seizure, and complex type of seizure increase the risk of recurrence of febrile seizure during hospital course. Further studies for assessing the long term effects of these factors on recurrence will determine children who need more medical attention in the future.
