بررسی اثرات اندازه تومور و نوع رادیونوکلید بر درمانپذیری تومور در رادیوتراپی هدفمند

<p>زمینه: رادیوتراپی هدفمند یکی از روشهای مهم رادیوتراپی است که شامل استفاده از رادیونوکلیدهای ساطع کننده ذرات بتا برای تحویل دوز تابشی به سلولهای تومور میشود. از ویژگیهای مهم در این روش، برد ذرات بتای گسیل شده از رادیونوکلیدها و اندازه تومور میباشد که اثرات قابل توجهی بر درمانپذیری تومورها دارد. مواد و روشها: از شبیهسازی مونتکارلو و مدل ریاضی جهت بررسی ارتباط بین احتمال درمانپذیری تومور و اندازه تومور برای تومورهای درمان شده با I131 و Y90 استفاده شد. در این مدل فرض شد که رادیونوکلید بهصورت یکنواخت در تمامی تومور توزیع شده است. یافتهها: نتایج نشان میدهند که یک اندازه تومور بهینه برای درمان وجود دارد. برای هر اکتیویته تجمعی، احتمال درمانپذیری برای تومورهایی که قطرشان نزدیک به این مقدار بهینه باشد بیشتر است. ماکزیمم احتمال درمان برای I131 برای تومورهای با قطر حدود 5/3 میلیمتر اتفاق میافتد در حالیکه برای Y90 این ماکزیمم برای تومورهای با قطر حدود 5/3 سانتیمتر رخ میدهد. تومورهای کوچکتر از اندازه بهینه آسیبپذیری کمتری به پرتودهی رادیونوکلیدها دارند زیرا مقدار قابل توجهی از انرژی واپاشی فرار کرده و بیرون از حجم تومور انباشت میشود. تومورهای بزرگتر از اندازه بهینه بخاطر تعداد سلولهای بیشتر، درمانپذیری کمتری دارند. نتیجهگیری: در درمان به روش رادیوتراپی هدفمند با تک-رادیونوکلید، یک اندازه درمان بهینه وجود دارد. بنابراین پیشنهاد میشود که از رادیوتراپی هدفمند با تک رادیونوکلید برای درمان سرطانهای گسترشیافته که معمولاً دارای اندازههای مختلف هستند، استفاده نشود. استفاده از چند رادیونوکلید بهطور همزمان اثربخشی بیشتری نسبت به استفاده از تک رادیونوکلید میتواند داشته باشد. این نگرش استفاده از ترکیب رادیونوکلیدها با ویژگیهای تکمیلی، ارائه تدابیر جدید و افزایش بازدهی درمان تومور را میتواند میسر سازد.</p>
<p>Background: Targeted radiotherapy is one of the important methods of radiotherapy that involves the use of beta-emitting radionuclides to deliver a dose of radiation to tumor cells. An important feature of this method is the tumor size and the finite range of beta particles emitted as a result of radionuclide disintegration those have significant effects for the curability of tumors. Material and Methods: Monte Carlo simulations and mathematical models have been used to investigate the relationship of curability to tumors size for tumors treated with targeted 131I and 90Y. The model assumed that radionuclides are distributed uniformly throughout tumors. Results: The results show that there is an optimal tumor size for cure. For any given cumulated activity, cure probability is greatest for tumors whose diameter is close to the optimum value. There is a maximum value of curability that occurs at a diameter of approximately 3.5 mm for 131I. For 90Y maximum curability occurs at a tumor diameter of approximately 3.5 cm. Tumors smaller than the optimal size are less vulnerable to irradiation from radionuclides because a significant proportion of the disintegration energy escapes and is deposited outside the tumor volume. Tumors larger than the optimal size are less curable because of greater clonogenic cell number. Conclusion: With single radionuclide targeted radiotherapy, there is an optimal tumor size for tumor cure. It is suggested that single agent targeted radiotherapy should not be used for treatment of disseminated disease when multiple tumors of differing size may be present. The use of several radionuclides concurrently would be more effective than reliance on single radionuclide. This approach of using combination of radionuclides with complementary properties could hopefully prepare new measures and improve the efficiency of tumor therapy.</p>
