Background: Patient safety is one of the most important aspects of healthcare quality, and medical errors are considered to be the main challenges of the health system.
Materials and Methods: This study investigated medical errors and the factors affecting them in Kowsar Women's General Hospital in Urmia.
Results: Most of the 188 errors reported occurred in 2022 (45.2%) and during the night shifts (71 cases). A significant relationship was observed between the type/department of error and shift; night shifts had the most care errors (47.1%) followed by delivery (50%) and emergency (42.1%) departments. Residents were responsible for 66.5% of errors, temporary complications (44.3%) were the most common outcome, and care events (34.6%) were the most common type (P=0.01).
Conclusion: Errors mainly occurred during the night shift and at delivery, and operating room (care/surgery) and residents played a major role. Strengthening management, team coordination, and night shift supervision are essential to improve patient safety.
